Although maintaining and monitoring healthcare databases might be difficult, their efficiency is essential for ACO institutions’ organizational growth. Better healthcare solutions, especially in nursing informatics, could be made possible by artificial intelligence and cutting-edge information and communication technology (Robert, 2019).

Preventive health, patient experience, at-risk population health, and care coordination/patient safety are the four main areas of focus for the quality enhancements needed to join an ACO. To become an ACO, the hospital must demonstrate progress in these areas. To be able to track this information, the hospital’s Health Information Technology (HIT) system needs to be updated. The case manager has been notified by Karen Wellington, the Admissions and Discharge Director, that the hospital requires suggestions for enhancing HIT in order to monitor the data required to determine how to implement the required changes.

 

Meeting with patients who have not received enough preventative care, like Caroline McGlade, is the first step in improving HIT. In order to help the hospital make the required changes to become an ACO, the case manager should consider how the hospital could better track data in Caroline’s Electronic Health Record (EHR), which is not comprehensive. As a case manager, I also have to conduct interviews with different stakeholders that can offer pertinent details regarding the modifications that must be made to their HIT (Rai et al., 2021).

In my role as a case manager, I would speak with medical professionals who have direct patient contact. These healthcare professionals can offer insightful information about community problems and care barriers that impact the standard of care. They can assist in determining the type of data that must be monitored in order to gauge issues and advancements. For instance, clinicians can assist in identifying preventative care procedures like colonoscopies and mammograms that are not offered to patients.

The hospital can monitor advancements in preventive health, one of the primary areas needed to achieve ACO designation, by collecting data on these services.
