Brief of Strategie
I will suggest building relationships of trust, communication, courteous criticism, and honesty among group members to guarantee (NHS FPX 6004 Assessment 4) buy-in, support, and readiness. Organizations need to implement an evidence-based methodology that best fits each member of the group’s creative point of view. After this is accomplished, individuals will concentrate on the current tasks in an effort to successfully align with the organization’s goals (Sas et al., 2021). In order to maintain consistency throughout the entire process, I decided to test my idea with this particular group. I recruited an executive board member to help since their involvement will reaffirm to the hospital’s staff that all echelons of leadership are totally committed to this endeavor.
In order to prepare, navigate the organization, and make sure the policy complies with local, state, and federal regulatory agency requirements, the hospital’s public health nurses and public relations director will be essential (Evans, 2020). Involving local leaders in this endeavor is essential. A deeper comprehension of the requirements of the community can be obtained from leaders. Above all, the community leadership association demonstrates to residents and visiting patients that this updated policy is intended to promote community well-being and health.
PART- 2
Training Agenda
Objective
- Learning Objectives: To enhance the standard of care and results for our diabetes patients at Mercy Medical Center and the surrounding areas.
Identify Training Activities
- The training approach would involve retraining staff members via workshops for hospital employees, yearly trainings, and education seminars (Reaves, 2021).
- Gives patients and the hospital community a new reason to make testing accessible to them.
- Fosters a revitalized intent for patient care, volunteer work, and collaborations. The success of policy can be ensured by effective relationships with governmental agencies, other hospitals, and members of the community (Strasser & Strasser, 2020).
Policy Practice and Practice
- Boost diabetes screening and testing among patients and the community.
- Organize community health fairs and semi-annual diabetes testing clinics with the goal of raising community knowledge and involvement (Botiba et al., 2024).
- Incorporate a mobile home testing program so that patients who are unable to travel can be tested by nurses (Soska & Navarro, 2020).
- A policy is implemented to enhance the quarterly totals of the Mercy Medical Center Public Health Diabetes Testing Dashboard, which is currently not meeting expectations (Catley et al., 2022).
- The manner in which they work every day won’t change. There will be a rise in responsibilities and a continuous emphasis on providing attentive patient care.
- Groups will have the responsibility of enhancing patient education regarding the maintenance of diabetes and the rationale behind diabetes testing. The National Diabetes Education Program (NDEP) of the National Institutes of Health will provide training materials (White et al., 2020).
- The NDEP is a program designed to provide people the knowledge, abilities, and self-assurance they need to control their diabetes and lead healthy lives. By incorporating the NDEP training recommendations, staff members will be better equipped to introduce patients to suggested hospital services and help them feel balanced and like normal people (Kelley et al., 2020).
Justify the importance of change
- These adjustments are necessary since diabetes diagnoses are increasing, and putting this strategy into place will help with early identification, lower patient mortality, and lower medical expenses.
- Facts that highlight the significance of enacting policy changes:
- In 2020, diabetes or prediabetes will be diagnosed in 50% of American adults.
- 60 million Americans do not realize they are in the pre-diabetic stages of life (Zeceña, 2022).
- Between 2015 and 2020, there will be a 54% increase in Type I and Type II diabetes cases.
- Between 2015 and 2030, there will be a 38 percent increase in diabetes-related fatalities and a 53 percent increase in medical expenses.
Role of the group and Practice Guidelines
- For the introduction of a new policy to be successful, all stakeholders need to be in agreement.
- The value of collective projects
a) Patient engagement will rise when everyone in the group
