Mood disorder is one of the mental conditions that impact the ability of patients to function. The PMHNP must perform a comprehensive assessment of the mood disorders to formulate the most appropriate diagnosis and treatment interventions. P.K explains that she is concerned about the history of taking medications which she eventually stops taking them claiming that she does not require them. P.K explains that she had in the past, been hospitalized four times due to mental illness. P.K explains that she was admitted to the hospital during her teens because she had four to five days without sleeping. P.K was again hospitalized during the year 2017 because she tried to commit suicide. PK overdosed on Benadryl. P.K explained that she heard voices during that time. P.K was also admitted to hospital after she was found by the police officers dancing around with her
3 gaiter around in a field in her nightgown. According to P.K, her mother made up a study so that she can force her to get back and put up with her boyfriend. P.K explain the was told she has anxiety, bipolar disorder and depression disorder. Based on medication, P.K was given Zoloft in the past. However, he explains that Zoloft made her feel really "high". The medication also made her feel racing thoughts and he could not sleep. She also reported using Seroquel and Risperdal. However, this medication made her gain weight. P.K explain using Klonopin but this slows her down. P.K also reported signs of low energy, loss of interest, not wanting to get out of bed, and low motivation at some points in the past. P.K reported "creativity" in which she has too much energy and completes a lot of things. P.K explains that during the time of "creativity" she would not sleep for four to five days. P.K explains that he friends complain that she talks too much during times of creativity. P.K explain that her mind is scattered during "creativity" and she engaged in sex with different individuals during this time because she feels excited and thrilled to explore new sexual situations with other persons. After all, it helps to keep her mood high. P.K explained that these episodes go on for one week and get 5-6 hours of sleep during these times. Past Psychiatric History General Statement:PK explain that she has depression and was, therefore, prescribed medications that she does not like. She is cooperative throughout the interview. She is attentive, understands, and responds to the prompts. Family History:PK reported that her mother tried to commit suicide but was unsuccessful. She further reported that her mother had Bipolar disorder. The family history also indicates that his father was put in prison because of drugs. PK has not met her father for several years-about ten
4 or eight years. Her brother also has mental health conditions (schizophrenia) but has never been treated or diagnosed. Caregivers (if applicable):At present, PK stays with her mother and sometimes with KP's boyfriend. Substance Current Use:PK has reported substance use. She explained that she once tried cannabis. She never liked cannabis as it made her paranoid.Other instance of substance use includes using one pack of cigarettes every day. Even though she denied using alcohol, she reported her last use of alcohol at the age of 19. PK denied using cocaine. Hospitalizations: PK has reported several instances of hospitalizations. She had been admitted because of possible mental health problems four times. She was also admitted due to a suicidal attempt back in 2017. She was admitted due to an overdose of Bendery.She was also found misbehaving and a police officer picked her and she was hospitalized. Current Medications:PK is currently taking birth control medication. PK is taking thyroid medication. Medication trials:PK has tried different medications for her anxiety and depression disorder.
